Dr. Thompson's review repeats the conclusions of MSLT-I and those authors' interpretaton of the data to suggest a survival and prognostic benefit to SNB when another view supported by the results is that the same data show no benefit. In fairness, Dr. Thompson mentions that "the final (long-term) results of ... MSLT-I ... are not yet available and that any conclusions drawn now may require modification when long-term trial results have been analyzed." He adds that "What is not yet clear is whether CLND is necessary in all patients with positive SNs ...." He concludes that "At this stage it appears that metastatic tumor volume in the SN and the distribution of tumor within it are the most important determinants ... of the need for CLND." This point is where we may find agreement within the controversy over SNB.
