This is a small-sample study investigating the use of diffusion-weighted MRI in acute herpes simplex encephalitis. DWI was applied in three patients and compared with the results of conventional MRI. DWI appears to demonstrate cortical diffusion abnormalities as the initial or most sensitive sign of encephalitis.

Thirteen patients with acute encephalitis were evaluated for late changes on MRI during a period of 18 months after the initial infection. Radiological features appeared to remain stable during the study period. Moreover, six of nine seropositive individuals showed discrete, small, hyperintense lesions in the white matter very similar to those in patients with encephalitis.

This study investigated short-term and long-term outcomes in six children with EV71 encephalitis and who had survived the acute stage characterized by pulmonary edema. MRI disclosed signal changes in the posterior pons and medulla. DWI was superior to conventional MRI in terms of the early detection of hyperintense symmetrical abnormalities in the posterior medulla.

This is one of the largest imaging studies on Japanese encephalitis comparing the clinical and imaging features of 30 children and 37 adults. Dystonia was more common in children; all other clinical signs occurred at the same frequency in adults and children. The results of cranial computed tomography and MRI did not differ between the two age groups.

The findings with magnetic resonance spectroscopy were suggestive of inflammation in the early stage without signs of neuronal loss. In the late stage of the disease, magnetic resonance spectroscopy revealed signs of neuronal degeneration.

    • Mader I, Herrlinger U, Klose U, et al. Progressive multifocal leukoencephalopathy: analysis of lesion development with diffusion-weighted MRI. Neuroradiology 2003; 45:717-721. This paper presents data on DWI in two patients with PML. The extent of abnormal diffusion correlated with the speed of clinical deterioration. DWI appeared to be more sensitive than conventional MRI in the identification of areas of progressive disease.

This study examines whether DWI with ADC maps is valuable in the discrimination of lesions due to Toxoplasma encephalitis (n = 13) from PCNSL lesions (n = 8). Toxoplasma lesions were characterized by significantly greater ADC ratios compared with those of PCNSL. ADC ratios greater than 1.6 were observed only in Toxoplasma lesions.
