The authors surveyed the causes of chorea in a consecutive series of 51 patients in a tertiary referral centre. After the exclusion of five patients with Huntington's disease and one with neuroacanthocytosis, all but three patients had the aetiology of their movement disorder determined. Stroke, found in 21 subjects, was by far the most common cause.

In this prospective study the authors carefully followed up four patients with chorea related to perinatal hypoxic-ischaemic lesion treated with double-burst stimulation of the globus pallidus internus. The mean improvement at the last visit, just 23% on an objective rating scale, failed to reach statistical significance. This study highlights the theory that chorea related to cerebral palsy is refractory to medical and surgical interventions.

Whereas Parkinsonism or dystonia were seen in 5% of SC patients treated with neuroleptic agents, these complications were not seen in matched TS patients exposed to similar chlorpromazine-equivalent dosages. This is another clinical difference between TS and streptococcus-induced neuropsychiatric syndromes.

Neuropsychiatric syndromes, present in 57.3%, were strongly associated with antiphospholipid antibodies. Chorea, however, was observed in less than 2% of patients.

This is a report of one patient with oral contraceptive-induced chorea who was found to have circulating anti-basal ganglia antibodies. Although of debated biological significance, these antibodies are associated with SC. This study does not prove that the chorea of the patient was caused by an immune mechanism. However, it strongly supports the concept that this form of chorea is related to rheumatic fever.
