Genetics, clinical phenotype, and molecular cell biology of autosomal recessive hypercholesterolemia
-
Soutar AK, Naoumova RP, Traub LM. Genetics, clinical phenotype, and molecular cell biology of autosomal recessive hypercholesterolemia. Arterioscler Thromb Vasc Biol 2003; 23:1963-1970. This review provides useful information on the history of the genetics of ARH from the first clinical description of the condition, the recognition of a new cellular phenotype and mapping the gene on chromosome 1p35-36.1 to gene cloning. New insights into the role played by ARH protein in gathering the LDL receptor into forming endocytic carrier vesicles are discussed. The ARH clinical phenotype is critically compared to that of homozygous FH patients.

Thompson GR. LDL apheresis. Atherosclerosis 2003; 167:1-13. This comprehensive review summarizes current knowledge on LDL apheresis starting from the history of the procedure and its application as safe and reliable treatment for patients with homozygous FH, where there is clear evidence of benefit. The possibility of widening the indications for LDL apheresis to include patients with post-transplant donor disease, stroke and prevention of re-stenosis after coronary angioplasty is examined and the need for evidence from controlled trials is also discussed.

Atherosclerosis 2003; 171:273-279. This randomized, double-blind, cross-over trial involving 27 patients with homozygous FH showed that avasimibe, an acyl coenzyme A-cholesterol acyltransferase inhibitor, only modestly (by around 4%) enhances the lipid-lowering effect of atorvastatin 80 mg. No significant lipid changes resulted from monotherapy with avasimibe.
