Older women who undergo postmastectomy reconstruction reported a high rate of satisfaction with the procedure. In only 16% of the study group, however, was reconstruction discussed with them at time of their cancer diagnosis. In nearly 50% of patients, delayed reconstruction was performed. This illustrates the disparity in referral patterns or access to care in the elderly breast cancer population.

An analysis of long-term complications, aesthetic outcomes, and patient satisfaction. Plast Reconstr Surg 2006; 118:832-839. This large-series evaluates long-term outcomes following two-stage implant reconstruction. Based on the results of this series, it appears that patient satisfaction does not correlate with the development of a complication or with the overall aesthetic result. This suggests that the factors that influence patient satisfaction with reconstruction remain unclear.

Garvey PB, Buchel EW, Pockaj BA, et al. DIEP and pedicled TRAM flaps: a comparison of outcomes. Plast Reconstr Surg 2006; 117:1711-1719. In this study, harvest of the DIEP flap resulted in fewer abdominal hernias compared with after harvest of the pedicled TRAM flap. The rate of abdominal bulge, however, was similar in the two patient subgroups. Further studies analyzing the true benefit of perforator flaps in terms of complications relative to free TRAMs and pedicled TRAMs with abdominal wall reinforcement (i.e. mesh/AlloDerm) are needed.

Alderman AK, Kuzon WMJ, Wilkins EG. A two-year prospective analysis of trunk function in TRAM breast reconstructions. Plast Reconstr Surg 2006; 117:2131-2138. Based on the results of this prospective investigation, it appears that there is no difference in objective trunk function in patients who undergo pedicled TRAM flap reconstruction and those who undergo free TRAM flap reconstruction. It appears, however, that truncal function is decreased in patients following harvest of a TRAM (pedicled or free) flap compared with those who undergo implant based reconstruction alone.

Alderman AK, Kuhn LE, Lowery JC, Wilkins EG. Does patient satisfaction with breast reconstruction change over time? Two-year results of the Michigan Breast Reconstruction Outcomes Study. J Am Coll Surg 2006; 204:7-12. Using a patient-reported outcomes instrument, these authors determined that at 2 years after reconstruction, general satisfaction was the same in patients who had a TRAM (or related) flap and those who had implant based reconstruction. Patient-reported satisfaction with their aesthetic result was higher, however, in patients who underwent autogenous tissue reconstruction.

Saulis AS, Mustoe TA, Fine NA. A retrospective analysis of patient satisfaction with immediate postmastectomy breast reconstruction: comparison of three common procedures. Plast Reconstr Surg 2007; 119:1669-1676. This is an interesting study that demonstrates a correlation between decreased patient satisfaction and patient perceptions of poor preoperative counseling and inadequate information about options. The results of this study demonstrate the importance of the decision making process in achieving treatment success.

Psychological factors predict patient satisfaction with postmastectomy breast reconstruction
-
Patients with a history of psychiatric disturbance and anxiety have significant effect on patient satisfaction after reconstruction. It is crucial to develop methods to predict which patients will be at risk for this and to tailor their treatment experience accordingly to prevent it
