Older women who undergo postmastectomy reconstruction reported a high rate of satisfaction with the procedure. In only 16% of the study group, however, was reconstruction discussed with them at time of their cancer diagnosis. In nearly 50% of patients, delayed reconstruction was performed. This illustrates the disparity in referral patterns or access to care in the elderly breast cancer population.

An analysis of long-term complications, aesthetic outcomes, and patient satisfaction. Plast Reconstr Surg 2006; 118:832-839. This large-series evaluates long-term outcomes following two-stage implant reconstruction. Based on the results of this series, it appears that patient satisfaction does not correlate with the development of a complication or with the overall aesthetic result. This suggests that the factors that influence patient satisfaction with reconstruction remain unclear.

Based on the results of this prospective investigation, it appears that there is no difference in objective trunk function in patients who undergo pedicled TRAM flap reconstruction and those who undergo free TRAM flap reconstruction. J Am Coll Surg 2006; 204:7-12. Using a patient-reported outcomes instrument, these authors determined that at 2 years after reconstruction, general satisfaction was the same in patients who had a TRAM (or related) flap and those who had implant based reconstruction. Patient-reported satisfaction with their aesthetic result was higher, however, in patients who underwent autogenous tissue reconstruction.

Plast Reconstr Surg 2007; 119:1669-1676. This is an interesting study that demonstrates a correlation between decreased patient satisfaction and patient perceptions of poor preoperative counseling and inadequate information about options. The results of this study demonstrate the importance of the decision making process in achieving treatment success.
