Neurology 55, 281-288 (2000). • The original medical hypothesis that links visual hallucinations (VH) with dream overflow. The VH that coincide with daytime episodes of rapid eye movement (REM) sleep in patients who also experience post-REM delusions at night may be dream imagery. Psychosis in patients with Parkinson's disease (PD) may therefore reflect a narcolepsy-like REM sleep disorder.

Goetz CG, Stebbins GT. Mortality and hallucinations in nursing home patients with advanced Parkinson's disease. Neurology 45(4), 669-671(1995). •• The most quoted paper on mortality risk and VH. The study demonstrates the permanency of nursing home placement in advanced PD and the high mortality associated with such placement. It also documents the chronicity of hallucinatory behavior in these patients with advanced PD and reinforces our previously reported observations on the relationship between hallucinations and placement in chronic-care facilities.

Neurology 50, 515-517 (1998). • This study confutes the concept of dopaminergic psychosis. In the study, plasma levodopa levels changed significantly during the infusion protocol. All patients remained alert, and none developed VH. In conclusion, VH do not relate simply to high levels of levodopa or to sudden changes in plasma levels.

Fenelon G, Mahieux F, Huon R, Ziegler M. Hallucinations in Parkinson's disease: prevalence, phenomenology and risk factors. Brain 123(Pt 4), 733-745 (2000). •• This study determined the phenomenology, prevalence and risk factors of hallucinations in PD. It was demonstrated that three factors were independently predictive of formed VH: severe cognitive disorders, daytime somnolence and a long duration of PD. The main risk factor in treated patients is cognitive impairment, although sleep-wake cycle disturbances and possibly other factors related to the duration of the disease act as cofactors.

Neurosci. 9, 173-189 (2007) •• This article reviewed the history of visual hallucinatory syndromes and the eye dispute, together with advances in perceptual neuroscience that question core assumptions of our current approach. From a neurobiological perspective, three syndromes emerge that relate to specific dysfunctions of afferent, cholinergic and serotonergic visual circuitry and promise future therapeutic advances.

Disord. 20, 130-140 (2005). • The most quoted model of VH origin. The authors suggest that visual hallucinations should be considered as a dysregulation of the gating and filtering of external perception and internal image production. This new conceptual framework permits an anatomical view of visual hallucinations and suggests new, testable hypotheses regarding their pathophysiology and therapy.

Ravina B, Marder K, Fernandez HH et al. Diagnostic criteria for psychosis in Parkinson's disease: report of an NINDS, NIMH work group. Mov. Disord. 8, 1061-1068 (2007). •• The authors reviewed the existing literature on PD-psychosis (PDPsy) to provide criteria that distinguish PDPsy from other causes of psychosis. They proposed provisional criteria for PDPsy in the style of the Diagnostic and Statistical Manual of Mental Disorders IV-TR. PDPsy has a well-characterized temporal and clinical profile of hallucinations and delusions. PDPsy is associated with Lewy body pathology, imbalances of monoaminergic neurotransmitters and visuospatial processing deficits. These findings suggest that PDPsy may result from progression of the disease process underlying PD.
