Articles such as this, which report less than optimal outcomes after foam sclerotherapy, will unquestionably appear as this technique passes out of the hands of experts, into the general phlebologic community. Uurto and colleagues have, however, neatly encapsulated the major drawbacks to the large-scale incorporation of this promising technique. They include the lack of a stable and uniform source of foam preparations, volume limits, contraindications, and protocols. What this study confirms is a precipitous increase in thrombophlebitis when an unusually concentrated form of foam is employed. This complication might have been prevented by the use of lower concentrations or more prolonged compression.
