The significance of this exhaustive authoritative text lies much more in the impressive list of authors than in its quality. It is a tragedy, that many so-called consensus papers are not only patchwork from a point of style but also out of date when they are finally published. At least in the paragraphs on metabolic consequences of intestinal urinary diversion some of the perpetuated assumptions are obsolete or wrong.

5. Chang SS, Smith JA, Herrell SD, Cookson MS. Assessing urinary diversion experience in urologic residency programs: are we adequately training the next generation? J Urol 2006; 176:691-693. Important paper focussing on the issue of how to teach complex urinary diversion. The authors feel that alternative strategies to ensure proper training should be explored. There is a total lack of information on this crucial subject in the European literature.

Largest available series of ileal neobladders combining the experiences from Ulm and Bern (rate of orthotopic ileal neobladders in Ulm 1986-2006: 65%; in Bern 1984-2006: 51%). Comparsion of data from both centers confirms the assumption that no antireflux mechanism is needed in orthotopic reconstruction.

8. Wiesner C, Bonfig R, Stein R, et al. Continent cutaneous urinary diversion: long-term follow-up of more than 800 patients with ileocecal reservoirs. World J Urol 2006; 24:315-318. Largest series of continent cutaneous ileocecal reservoirs from the institution where the technique was devised originally at the beginning of the 1980s. The authors demonstrated that continent cutaneous urinary diversion is still a mandatory part of the surgical repertoire in urinary diversion.
