The significance of this exhaustive authoritative text lies much more in the impressive list of authors than in its quality. It is a tragedy, that many so-called consensus papers are not only patchwork from a point of style but also out of date when they are finally published. At least in the paragraphs on metabolic consequences of intestinal urinary diversion some of the perpetuated assumptions are obsolete or wrong.

Largest available series of ileal neobladders combining the experiences from Ulm and Bern (rate of orthotopic ileal neobladders in Ulm 1986-2006: 65%; in Bern 1984-2006: 51%). Comparsion of data from both centers confirms the assumption that no antireflux mechanism is needed in orthotopic reconstruction.
