    • Note: A direct correlation is often presumed to exist between the level of hospital expenses and that of health insurance premiums. Although some level of correlation undoubtedly exists, note that between 2000 and 2004, when the rate of increase in health insurance premiums experienced double-digit increases, the operating margins of top insurers increased from a range of 2 to 9 percent to a range of 9 to 13 percent (AHA, TrendWatch Chartbook 2006, supra n.1).
