International registry of laparoscopic radical cystectomy: First report on 392 patients

This study shows for the first time that in some patients with bladder cancer, LRC can provide equivalent oncological outcomes to open surgery. However, a word of caution; only eight patients had ≥ 5 years of follow-up. The subject is controversial, as another article from Simonato et al. cited in this paper, reported poorer oncological outcomes with LRC. There has also been a report of port-site metastasis after robotically assisted RC, and therefore this issue is not entirely resolved, and the effect of pneumoperitoneum on bladder cancer cells is unclear. Despite this, the risk of metastasis as result of the technical differences in minimally invasive cystectomy as opposed to open surgery are clearly very low. This paper has another very important message; LRC is an advanced technique with a high complication rate, even in experienced hands. Over half the patients had either an immediate or delayed complication.
