Significant changes in transrectal ultrasonic measurements of the prostate in relation to the degree of rectal wall distension
-
The author described a useful manoeuvre to improve the therapeutic ratio of cryoablation of the prostate. Lessening the pressure of the ultrasound transducer on the rectal mucosa increases the distance between the prostate and rectum. This allows for a more thorough freezing of the posterior prostatic tissue while avoiding rectal injury. The author notes that the anterior-posterior diameter of the prostate increases with this manoeuvre, and therefore additional cryoprobes might be needed to adequately freeze the entire prostate. An alternative technique to avoid the costly use of additional cryoprobes is to maintain firm pressure of the ultrasound transducer against the rectal mucosa for the entire planning process, and almost the entire freezing process. As noted by the author, this compresses the prostate.
