Arthroscopy 2006; 22:1085-1092. Full thickness cartilage defects refractory to nonoperative treatment were randomized into three treatment groups. There was a minimum 2-year follow up with a mean of 53 months. There was no significant difference between the groups in regards to outcome scoring. MRI findings suggest that microfracture may be the preferred initial treatment for these lesions.

This study reports on multiple approaches for osteochondral autografting, including their technique for triangular tibial wedge osteotomy and harvesting from the ipsilateral talar articular facet. Good or excellent results were obtained in 32/35 patients even after failed arthroscopic surgery. The incorporation of the osteochondral graft was monitored by radiographs and used as a guide to beginning full weightbearing.
