Lancet 2006; 368:379-385. A large, multicenter, randomized controlled trial comparing outcomes with IHD to CVVHDF in critically ill patients with AKI. No difference in outcomes was observed between study groups, but the authors did observe a progressive improvement in survival during the course of the study in the hemodialysis arm while outcomes in the continuous therapy arm were stable over time.

Data from studies that reported improved recovery of renal function in patients surviving AKI who were treated with CRRT compared with IHD were reanalyzed to adjust for the competing effect of mortality. The authors concluded that modality of therapy is not associated with improved recovery of renal function.

A single center randomized controlled trail demonstrating improved survival in patients with AKI in whom small solute clearance during CVVH was increased by the addition of dialysate flow (CVVHDF). This study supports the results of an earlier study by Ronco et al. [6] that augmented dosing of CRRT is associated with improved outcomes.

This paper provides a description of the study design of the ongoing ATN study which is evaluating an intensive versus a less intensive strategy for the management of RRT in AKI.
