This randomized study in 1000 patients with acute lung failure compared two fluid management strategies: liberal (similar to the current standard guide line in most centers) versus conservative. Liberally managed patients received more fluids and less furosemide than did those under conservative management. Although there were no differences in the 60-day mortality, lung function and duration of mechanical ventilation were significantly better in the conservative group, therefore this strategy is recommended.
