Since 1998 there is a regulation in Dutch healthcare that anyone who wants to be eligible for financed care needs to submit an application to the 'RIO'. The RIO is an institution that makes recommendations regarding the care that is needed in a given situation.

In policy and legislation concerning the indication and allocation of care the demands of the client are seen as the starting point in the process of individual needs assessment. A client is considered as an independent civilian who takes the responsibility for his situation himself as long as possible. When he is not able anymore to take care of his own situation, he can submit an application for professional care. A representative is expected to look after a client's interests in case he is not able to determine his position himself. In the official policy documents a model of autonomy is used that is based on the notion of informed consent; clients receive information and are supposed to formulate their needs independently and well-considered. If the clients are unable to make decisions for themselves, a representative should act on their behalf. On the basis of the client's wishes, the RIO will provide an objective expert judgement about the level of care needed. The statements of Carolien Franssen can be interpreted as a criticism against the consumer-directed care model.
