Outpatient tonsillectomy in children: A systematic review

Brigger MT, Brietzke SE. Outpatient tonsillectomy in children: a systematic review. Otolaryngol Head Neck Surg 2006; 135:1-7. This metanalysis of 17 articles related to outpatient tonsillectomy in children suggests the procedure is safe. Perioperative complication rates are estimated at 8.8% with an unplanned admission rate of 8%. Children under the age of 4 years were noted to be at an increased risk of complications.

The authors performed a trial on 20 fresh cadavers, evaluating temperature changes in the prevertebral fascia using electrocautery. They also did a retrospective and prospective review of cervical complications with a variety of techniques of adenoidectomy. The authors concluded that complications are uncommon, and not related to adenoidectomy technique or electrocautery.

In this retrospective chart review of 745 coblation and 1252 electrocautery tonsillectomies, an increased risk of postoperative neck pain was noted in the cautery/curette group. There was no difference in the risk of bleeding.
