A randomized study including 51 patients. The incidence of brain metastases was 3.8% after prophylactic WBRT versus 32% in the control group (P = 0.02); the survival increase did not reach the level of statistical significance (P = 0.13).

Twenty-eight patients with brain metastases originating mainly from lung carcinoma (n = 13), and melanoma (n = 4), located either in the pons (n = 17), the midbrain (n = 9), or the medulla (n = 2), were treated with radiosurgery. Only six patients underwent WBRT. Local control was achieved in 92% of the patients and death was related to tumour progression in only two patients. The procedure is considered safe by the authors.

Postradiosurgical imaging showed that 62% of the lesions decreased, 19% remained stable, and 19% eventually increased in size. Postradiosurgery MRI or CT scans were performed in only 14 patients and 21 brain metastases, however.

This retrospective analysis considers 110 patients with brain metastases of various origins 1 year or longer after radiosurgery: 70 patients were treated with radiosurgery plus WBRT and 40 with radiosurgery alone. Adding WBRT to radiosurgery improved local control and decreased intracranial relapse in patients with brain metastases of 2 cc or greater, peripheral dose 16 Gy or more, single metastases, nonradioresistant tumours, and lung cancer.

The study shows a significant benefit of surgery on the ability to walk (the primary endpoint, P = 0.001), the preservation of the ability to walk (P = 0.003), and the decreased need for corticosteroids and opioid analgesics. The patients were stratified according to institution, tumour type, ambulatory status, and spine stability. It is important to keep in mind that patients were included on the basis of MRI evidence of spinal cord displacement on the cervical and dorsal (not lumbar) level and that patients with multiple spinal compressions and radiosensitive tumours were excluded from the study.

It concludes that all studies provided only class III evidence. Increases in quality of life, survival, and time to progression in responders suggest that a subset of the treated population may benefit from intrathecal chemotherapy.
