The authors used linkage analysis in a large Greek pedigree segregating otosclerosis as a monogenic autosomal dominant trait, and excluded linkage to four known otosclerosis loci (OTSC1, OTSC2, OTSC3, and OTSC5), as well as to the COL1A1 and COL1A2 genes, indicating that otosclerosis involves at least five different genes.

Researchers demonstrate that osteoprotegerin is expressed at high levels within the soft tissues of the cochlea and is secreted into the perilymph and the surrounding bone, and may serve to inhibit active bone remodeling within the otic capsule, especially adjacent to the cochlea. This paper describes for the first time the extensive system of interconnected canaliculi within the otic capsule with openings into perilymph, thus providing a possible anatomic route through which the cochlear soft tissue may control the surrounding otic capsule.

The investigators demonstrate that osteoprotegrin knockout mice provide the best available animal model to date to study abnormal otic capsule remodeling that closely resembles otosclerosis. Unlike human otosclerosis, however, these mice do not develop stapes fixation, and they exhibit diffuse remodeling affecting the ossicular chain and the systemic skeleton.
