Investigators recruited 1170 women age 13-36 at high risk for chlamydial infection and followed them for 3 years for incident PID. Based on a multivariable analysis, risk factors for PID included: age at first intercourse of 15 years or less, current cervicitis, history of PID, annual income less than $20 000, smoking, medroxyprogesterone acetate use, and sex with menses.

This study investigates the controversy surrounding the association of bacterial vaginosis (BV) and PID. This was a multicentre study that collected serial vaginal swabs for Gram's stain and culture and correlated the findings with cases of PID. Carriage of non-BV-associated organisms did not alter one's risk for PID. Heavy growth of BV-association organisms resulted in an increased risk for PID (OR=2.0, CI=1.2, 3.5). When heavy growth was combined with a new sex partner the odds ratio increased to 8.8 (CI=1.1, 69.2).

This study investigated a novel diagnostic modality for PID in 15 women with suspected PID. Scintigraphy with 99mTc-HMPAO labeled leukocytes was used to detected PID with a reported 100% sensitivity, 90% specificity and overall accuracy of 93%. Positive scintigraphic uptake revealed circular areas of increased leukocyte accumulation on the pelvis consistent with inflammation. Use of this rapid and highly accurate diagnostic modality may assist the clinician in determining whether operative or conservative management is appropriate in difficult diagnostic cases where nuclear medicine services are readily available.

The PEACH study was an RCT of inpatient and outpatient therapy for PID. This analysis included expanded follow-up for a mean of 84 months of follow-up. Outpatient therapy did not adversely impact reproductive outcomes (that is, fertility, ectopic, PID recurrence, chronic pelvic pain). This was true even in subgroups of adolescents and women without a previous live birth.
