This multicenter study involving 3961 patients evaluated the subareolar injection site, confirming that the entire breast functions as one lymphatic unit.

Injection of (99m)Tc-labeled sulfur colloid the day before operation for breast cancer sentinel lymph node mapping is as successful as injection on the day of operation. Am J Surg 2004; 188: 685-689. The authors investigated the timing of radiocolloid injection and found no statistical difference in the sentinel lymph node identification rate in patients injected on the day before and those injected on the day of surgery.

This controlled clinical trial concluded that performing SNB the day after radiocolloid injection did not diminish the accuracy of predicting regional lymph node metastases.

This study reported a 97% successful identification rate of sentinel lymph nodes using an intraoperative subareolar injection of radioisotope. These data support immediate staging of the axilla by SNB using this technique.

An important paper from the MD Anderson Cancer Center examining factors associated with false-negative sentinel nodes in breast cancer.

Prerandomization surgical training for the National Surgical Adjuvant Breast and Bowel Project (NSABP) B-32 trial: A randomized phase III clinical trial to compare sentinel node resection to conventional axillary dissection in clinically node-negative breast cancer. Ann Surg 2005; 241:48-54. NSABP conducted a rigorous training program, including the proctoring of surgeons by a senior study investigator for the B-32 trial. They accomplished high-quality SNB performance with fewer than 10 cases per surgeon before allowing surgeons to enter patients onto the trial.

J Clin Oncol 2005; 23: 2694-2702. This is the largest study of SNB with neoadjuvant chemotherapy. Patients had SNB as part of surgeon SNB training outside the primary study, and NSABP investigators aggregated the findings. Although the study was done with a variety of SNB techniques, the report demonstrates the accuracy of SNB with neoadjuvant therapy.

This paper makes strong recommendations but is based on a relatively small number of patients.
