This report is the first demonstration that growth, development and positive nitrogen balance could be achieved by long-term parenteral nutrition (PN) in animals and man. Six male beagle puppies fed entirely by IV for 72-256 days with hypertonic solutions infused into their superior vena cavas matched their orally fed controls in weight gain, skeletal growth, development and activity. Thirty patients with chronic complicated gastrointestinal disease had been supported exclusively by central venous infusions of 2,400-4,500 calories per day for 10-200 days with PN. Metabolic studies in the first 6 patients with long-term PN demonstrated significant positive nitrogen balance in the presence of a variety of generally catabolic clinical situations. The most dramatic result of these studies was the first documentation of normal growth and development of a human infant in a 1.8 kg female with near-total small bowel atresia fed entirely by PN for 44 days. This study also represents the direct clinical application of knowledge, experience and technology gained in the experimental animal laboratory to the successful resolution of a complex situation with no previously existing solution. This report describes the course, management, and metabolic data of the first human infant to exhibit normal growth, development and activity while being fed long-term entirely by IV with the central venous infusion of hypertonic nutrients into the superior vena cava for 44 days. The infant, who weighed 2,300 gm at birth, had atresia of the small bowel from the ligament of Treitz to within 3 cm of the ileocecal junction and segmental atresia of the colon. Following 2 abdominal operations, her weight declined to 1,816 gm despite vigorous conventional IV nutrition support. Normal growth and development accompanied increases of 5.0 cm in head circumference, 6.3 cm in length, and 1,447 gm in weight during the 44 day period when the infant was nourished exclusively by central vein. These results were unprecedented and had a subsequent significant impact on the operative management and nutrition support of infants and children with catastrophic congenital anomalies. Although Aubanic first reported the technique of percutaneous subclavian venipuncture to achieve rapid transfusion in severely injured patients in 1952, and others reported successful use of his technique subsequently in Europe and South America, this avenue of central venous access was not used in the United States until reported by these authors. Even in this series of more than 100 critically ill patients, in whom access to the subclavian vein was achieved by infraclavicular percutaneous puncture and catheterization, the purpose of the procedure was to monitor central venous pressure as an aid in determining the adequacy of fluid replacement and not for administration of IV colloid or crystalloid fluids. However, their description of the technique, safety and efficacy of percutaneous subclavian vein access paved the way for the use of percutaneous infraclavicular subclavian catheterization for the continuous infusion of hypertonic nutrient solutions into the superior vena cava in the early development of PN. After Scribner abandoned the use of arteriovenous shunts and fistulas for infusing hypertonic PN solutions as his version of the "artificial gut" because of the prohibitive incidence of clotting, his Nephrology Fellow, Broviac, developed a silicone rubber catheter with a Dacron-cuff for long-term implantation for PN in 18 hospitalized patients, 9 of whom continued their infusions subsequently at home. Because the internal diameter of this catheter was too small and often clotted when used for infusing blood products or for sampling blood, Hickman increased the internal diameter of the catheter from 0.22 mm to 0.32 mm and increased the thickness of the catheter wall, originally for the long-term treatment of 75 randomized leukemia and aplastic anemia patients undergoing bone marrow, transplants. Later, he designed a double lumen version of this more durable and reliable catheter which allowed infusion of PN through one lumen, and blood sampling, or infusion of other solutions, through the other lumen. A triple lumen version of the catheter increased its versatility further by providing a separate lumen for each of these functions. The Hickman catheter continues to be a mainstay of long-term central vascular access for ambulatory home PN patients to this day. Cochrane Library Level 2

Between 1974 and 1983, 133 patients with a wide variety of complex conditions adversely affecting the functions of their gastrointestinal tracts were fed by ambulatory home PN for a cumulative period of >100 patient-years. In the acute group, 82 patients were treated with percutaneously placed infraclaviculoar subclavian catheters, and in the chronic group, 51 patients were treated with implanted silicone rubber, Dacron-cuffed catheters for a cumulative total of 38,939 patient days. Of the 125 implanted catheters, 15 were placed in the superior vena cava and 10 in the inferior vena cava for an average duration of 250 catheter-days, the longest single catheter remaining in situ for more than 5% years. Catheter-related sepsis occurred 33 times with the chronic implanted catheters, or once every 2.6 catheter-years. Of the 114 catheters placed percutaneously in the superior vena cava via a subclavian vein for an average duration of 68 days, the longest single catheter remained in situ for 213 days. Catheter-related sepsis occurred in this acute group 7 times, equivalent to 1 episode every 3 catheter years. One remarkable patient had been maintained exclusively by PN for the entire 13 years of his life at the time of the report. Cochrane Library Level 3
