Villena V, López-Encuentra A, García-Luján R, et al.: Clinical implication of appearance of pleural fluid at thoracentesis. Chest 2004, 125:156-159. A large number of patients were prospectively studied following rigorous norms. The most frequent cause of watery fluid was transudate, but most transudates were classified as serous effusions.

Is it meaningful to use biochemical parameters to discriminate between transudative and exudative pleural effusions?
-
Romero-Candeira S, Hernández L, Romero-Brufao S, et al.: Is it meaningful to use biochemical parameters to discriminate between transudative and exudative pleural effusions? Chest 2002, 122:1524-1529. In 249 consecutive patients prospectively studied, the criteria of Light et al. [9] were superior to clinical presumption for separating transudates from exudates. In patients receiving diuretic therapy, the protein gradient showed similar accuracy and a lower cost than the albumin gradient.

Agostoni E, Bodega F, Zocchi L.: Albumin transcytosis from the pleural space. J Appl Physiol 2002, 93:1806-1812. After demonstrating the occurrence of albumin transcytosis from the pleural space of anesthetized rabbits, the authors signal that the role in pleural fluid turnover of lymphatics seems to have been overemphasized and that transcytosis may contribute a relevant role in the removal of protein and liquid from the pleural space.
