Retrospective study, using multi-variate analysis, demonstrating a significant increase in bleeding after endoscopy in patients on anticoagulation therapy (coumadin) compared with antiplatelet therapy. Although the analysis was performed on patients after polypectomy, the results can be applied to most endoscopic therapeutic interventions.
2. Chan FKL, Ching JYL, Hung LCT, et al. Clopidogrel versus aspirin and esomeprazole to prevent recurrent ulcer bleeding. N Engl J Med 2005; 352:238-244. Excellent prospective randomized trial that demonstrates the increased bleeding risk associated with clopidogrel compared with aspirin plus esomeprazole in patients with healing gastric ulcers. It is a landmark study that should be in everyone's library.
3. Standards of Practice Committee. ASGE guideline: the management of low-molecular-weight heparin and nonaspirin antiplatelet agents for endoscopic procedures. Gastrointest Endosc 2005; 61:189-194. An excellent review which updates the reader on management of low-molecular-weight heparin and nonaspirin antiplatelet agents at the time of endoscopy. Data from the literature are nicely summarized.

7. Mui L-M, Teoh AYB, Enders KW, et al. Pre-medication with orally administered midazolam in adults undergoing diagnostic upper endoscopy: a double-blind placebo-controlled randomized trial. Gastrointest Endosc 2005; 61:195-200. An intriguing double-blind, randomized trial that demonstrates the impact of premedication with midazolam on patients' tolerance of endoscopy. The study was performed in China; therefore, its generalizability to other cultures may be limited. Still, it is worth a look.

This review provides an update on the basic pharmacology and pharmacokinetics of sedatives and analgesics. Strongly recommended for anyone interested in understanding the basics of light sedation.

Only available study that compares surgery with stenting in the palliation of patients with malignant gastric outlet obstruction. Although the study was retrospective, data and analysis make for an excellent discussion. Strongly recommended for review.
