    • This can be seen as a variant of "medicine from below" in which the clinical encounter is viewed by the physician rather than the patient. See Roy Porter, "The Patient's View: Doing Medical History from Below," Theory & Soc., 1985, 14:175-98.

    • What do men have to do with it? Fathers and mid-twentieth-century childbirth
    • Two technical choices made for this article should be recognized: (1)1 have elected to designate physicians and doctors as "he," which is as it was for the practitioners who lectured and published in this area. (2) The parent has been essentially equated with the mother, who appears to have been the main parent involved in the clinical encounter - or at least the parent described in pediatric texts. The subject of fathers is an important one, which will need separate investigation. See Judith Leavitt, "What Do Men Have to Do with It? Fathers and Mid-Twentieth-Century Childbirth," Bull. Hist. Med., 2003, 77:235-62, for a similar separation of male and female experiences of childbirth.

    • There was a great deal of interchange between American, British, and Scottish pediatric authors who contributed chapters to each others' texts. The early foundling hospitals were begun in New York: "Virtually every name that was to become prominent in pediatrics in New York City - Abraham Jacobi, Job Lewis Smith, Luther Emmett Holt, John Howland, William P. Northrup, Joseph O'Dwyer - was associated with the medical board of one or more of these institutions" (Peter English, "Pediatrics and the Unwanted Child in History: Foundling Homes, Disease, and the Origins of Foster Care in New York City, 1860-1920," Pediatrics, 1984, 73:699-711, on p. 704).

He was head of pediatrics at Johns Hopkins from 1926, and "during this period special clinics were established at the Harriet Lane Home by Edwards A. Park" (Cone, History [n. 3], p. 202). There were initially four special clinics, including the psychiatric clinic, headed by Leo Kanner.
