This paper determines how anesthesia level affects the degree to which the laryngeal adductor reflex is elicited by superior laryngeal nerve stimulation. The laryngeal adductor reflex becomes life-threatening when there is bilateral vocal fold closure causing airway obstruction, At 1.0 minimal alveolar concentration, the contralateral vocal fold did not close and unilateral vocal fold closure was weakened, reducing the possibility of airway closure. The results provide an understanding of the anesthetic effects on laryngospasm during extubation and may explain why airway protection is reduced after prolonged sedation.

This is a careful study of the cortical to subcortical efferent connections in the rhesus monkey. Using electrical stimulation, the authors identified the cortical region producing vocal fold closure. They injected an anterograde tracer that could be used to identify all those subcortical regions labeled by the tracer. The authors confirmed the fact that there are no direct connections to the laryngeal motor neurons in the brainstem from the cortex. The results suggest that only humans have fine direct cortical control of the laryngeal motor neurons, which may provide the necessary, fine rapid control of the laryngeal muscles needed for speech. A previous study by the same authors examined cortical to cortical connections in the same species.

This was a study in normal volunteers examining the effects of a bilateral block of the superior laryngeal nerve using bupivacaine in the paraglottic compartment. The effects of laryngeal sensory blockade on swallowing were evaluated using video fluoroscopy to identify penetration and aspiration, which were significantly increased with anesthesia versus saline injection. The effects were greater than previous studies using different anesthesia conditions and more rigorous evaluation techniques. The results confirm the important role of sensory feedback for the pharyngeal phase of swallowing but leave the mechanism for this finding undetermined. The authors suggest that it may be reduced laryngeal motor neuron drive after sensory blockade that allows for the reduction in airway protection.

Axonal transport mechanisms have been implicated in some genetic forms of amyotrophic lateral sclerosis, Kennedy disease, and hereditary sensory motor neuropathy (Charcot-Marie-Tooth disease).

A neuromuscular implant for chronic stimulation of the posterior cricoarytenoid muscle was placed in six patients with bilateral vocal fold paralysis and resulted in successful decannulation of three patients, which is a modest but exciting result with the first approach that will provide these patients with an adequate airway without compromising voice production.

The authors used a postcricoid surface electrode similar to the one developed by Guidi et al. more than 20 years ago to monitor activity in the posterior cricoarytenoid muscle while stimulating with a disposable recurrent nerve stimulator. This electrode seems smaller and more pliable than the Guidi version. Because of the low rate of RLN injury, a larger sample than the 54 patients reported here need to be studied to evaluate the validity, reliability, and sensitivity of this monitoring technique.

This is a large, prospective series of patients evaluated using neuromotor monitoring, which was able to provide estimates on the incidence of RLN injury in different types of thyroid surgery. The article concludes that neuromotor monitoring should be routine in cases of repeat surgery or malignancy.

This is the first report of others using the Berke selective denervation and reinnervation technique in patients with adductor spasmodic dysphonia. The surgery was performed in six patients and was successful in five of the six who had improved voice function and no longer required Botulinum toxin injections for symptom control.
