This huge retrospective study finds a surprisingly low number of patients with peripheral eosinophilia compared with other series. Nevertheless, it dramatically illustrates the poorer postoperative result achieved in patients with serum eosinophilia compared with those without serum eosinophilia. The authors advocate that these patients be counseled preoperatively for the likelihood of continued symptoms postoperatively and need for revisional surgeries.

The elegant histochemistry nicely demonstrates the elevation of cystinyl leukotriene receptors as well as a downregulation of these receptors with topical aspirin desensitization in patients with aspirin-exacerbated ECRS, not seen in the patient's nonaspirin-sensitive counterparts. The authors conclude that the elevated expression of CysLT1 receptor in aspirin-exacerbated ECRS is probably fundamental to the pathogenesis of aspirin sensitivity and to the mechanism of aspirin desensitization.
