Contact Dermatitis 2004; 51: 13-19. The authors tested a large series of patients with drug-induced periocular dermatitis. In addition to standard allergens, eyedrops should be tested to determine the accurate compounds responsible for atypical blepharitis.

A meta-analysis aimed at determining from the literature the influence of topical treatments on filtration surgery. Most studies consistently described inflammatory infiltrates in the conjunctiva that could favour further fibrosis. However, no definitive conclusions can be drawn from those studies.

The authors showed that conjunctival epithelium undergoes increased apoptosis in reaction to eyedrops. This may be a direct consequence of toxic drugs/preservatives, or may result from the release of inflammatory cytokines.

26. Manni G, Centofanti M, Oddone F, et al. Interleukin-1beta tear concentration in glaucomatous and ocular hypertensive patients treated with preservative-free nonselective beta-blockers. Am J Ophthalmol 2005; 139:72-77. A cross-over study bringing major information: first a proinflammatory cytokine is stimulated in the tears of patients with antiglaucoma medications; second, this increased cytokine secretion was only found with preserved timolol, which further demonstrates the role of preservative in inducing conjunctival inflammation.
27. Pisella PJ, Debbasch C, Hamard P, et al. Conjunctival proinflammatory and proapoptotic effects of latanoprost and preserved and unpreserved timolol: an ex vivo and in vitro study. Invest Ophthalmol Vis Sci 2004; 45:1360-1368. The first ex-vivo study, performed using a technique of flow cytometry in impression cytology specimens, which showed the overexpression of immune markers in eyes receiving preserved beta-blockers compared with unpreserved timolol. In addition, the study showed a lower immune activation in eyes treated with a prostaglandin analogue, and comprised an in-vitro part that demonstrated a low toxicological profile of the prostaglandin despite its higher concentration in benzalkonium chloride.

Ophthalmology 2004; 111:2186-2192. A similarly designed case-control study that demonstrates the overexpression of IL-6, IL-8 and IL-10 in the conjunctiva of patients receiving antiglaucoma drugs.
