Older adults can maintain 25(OH)D but need more vitamin D to overcome HPT associated with their diminishing renal function. The authors emphasize the importance of intrinsic production of active vitamin D in parathyroid glands and that serum levels of 25(OH)D do not necessarily reflect the vitamin D deficiency.

Comparative genomic hybridization, loss of heterozygosity (LOH) studies, and MEN 1 gene mutation analysis were performed on five patients with multiglandular pHPT, each of which contributed with two parathyroid tumors for analysis. Genetic changes were different in the two tumors from the same patient. It is likely that genetic changes disclosed in these tumors are secondary, and that the tumors arise primarily because of other factors. Genetic factors could be germline mutations in unknown low-penetrance genes, and changes affecting the calcium sensing ability. Epigenetic factors could include DNA methylation or histone acetylation/deacetylation. Physiologic factors could involve ionized calcium or phosphate changes, or decrease in vitamin D activity.

Members of a family with unusual inactivating mutation in the intracellular part of the calcium receptor (CAR) have displayed similar clinical symptoms as patients with pHPT, although involvement of CAR should have suggested that the family belong to the familial benign hypercalcemia entity. The family thus represents a combination of pHPT and familial benign hypercalcemia. Parathyroid tumors of this family had similar allelic losses as sporadic parathyroid tumors but also frequent LOH on chromosomes 12q and 7q, indicating acquired sensitivity to secondary genetic hits. Two family members had developed adenoma-like nodules with the highest LOH frequency and a similar pattern of allelic loss in two glands from the same individual.

The authors report the experience in 239 patients; mean operative time was 40 minutes, short-term cure rate was 98%, and the complication rate, including transient hypoparathyroidism, was 3.7%. Conversion to bilateral operation occurred in 8% of patients. The authors claim that MIVAP can be learned by any experienced parathyroid surgeon and may provide a similar decrease in operative time and improved patient comfort as the open minimally invasive approach. Localization with both MIBI and ultrasonography has improved patient recruitment and success rate. The operation can be more difficult with glands located higher in the neck, or with ectopic posterior location. For such glands, combination with the method described by Henry et al. [8] is recommended.

The main advantage was a marked reduction in hospital stay and a 50% reduction in hospital charges. Surgery 2003, 133:32-39. Altogether, 688 patients with pHPT were subjected to conventional bilateral neck exploration, without pre- or intraoperative guidance. Single adenoma was found in 78.8% of patients, multiglandular disease in 14.2% (of which 3.2% had multiple adenomas), and parathyroid carcinoma in 1.5%. The cure rate assessed by serum calcium and PTH levels after 3 and 12 months of follow-up was 97.7%. Mean operating time for the entire series was 65 minutes, and decreased to 35 minutes when patients with adenomas were selected. The authors calculated that the total cost for a patient undergoing exploration for single adenoma was one third less than the cost of a scan-directed minimally invasive operation.
