This is the largest series in the literature documenting the results of atlantoaxial fusion with C1 lateral mass screws. The authors treated 160 patients during a 14-year period, and successful stabilization was achieved in 157. There were no neurologic, vascular, or infectious complications.

The authors studied the morphometry of 50 dried atlas specimens and used their data to develop a modification of the technique of Harm and Melcher [2] for placing lateral mass screws. The thickness of the posterior arch at the thinnest part of the vertebral artery grew was leas than 4 mm in four cases (8%). This dimension has been reported to be even smaller by other authors (see text).

The dimensions of the C1 lateral mass were adequate in 10 of 10 cases (20 sides) to allow 3.5 mm screws to be placed without impinging on the vertebral artery or the spinal canal. C1-C2 transarticular screws and C2 pedicle screws were anatomically feasible in 94% and 91% of cases, respectively, when CT software was used to plan idealized trajectories and a minimal (0.5 mm) margin for error was accepted.

Spine 2003, 28:E461-E467. This is case report showed impingement of the ICA by the tip of a well-positioned C1-C2 transarticular screw. Anatomic studies showed that at times the ICA can be within 1 mm of the ideal exit point of a bicortical C1 lateral mass screw or C1-C2 transarticular screw. Contrast-enhanced CT is recommended preoperatively in every case in which a C1-C2 transarticular screw or C1 lateral mass screw may be inserted.
