A comparison of two surveys carried out a decade apart, revealing dramatic increases in prison facilities and in the prison population. The authors make a strong case for the growth and increased availability of services inside prison.

Br J Psychiatry 2003; 182:299-302. Excellent literature review and outline of a practical policy designed to ensure patients' rights. The policy put forward is suitable for use only in prisons with established health-care centers supported by a system integrated with NHS psychiatrists.

The authors' opinion is that 'the current system is collusive and dishonest: the championing of equivalence (a noble idea) enables us to feel better about the reality of a failing system of hospital transfers for mentally ill prisoners'.
