In uncontrolled patients with 30 mg/day nifedipine, the efficacy of 60 mg/day was double after bedtime as compared to morning administration of the drug. Moreover, bedtime administration of nifedipine GITS reduced the incidence of edema by 91% and the total number of secondary effects by 74% as compared to morning dosing.

Results indicate that, in elderly hypertensive patients, mainly characterized by a diminished nocturnal decline in blood pressure, dosing time with valsartan might be chosen at bedtime, for improved efficacy during the nocturnal resting hours, and the potential reduction in cardiovascular risk associated with a normalized day/ night blood-pressure ratio.

Results from this trial in a selected population of non-dipper patients indicate that the day/night blood-pressure ratio was unchanged after valsartan on awakening. This ratio was significantly increased when valsartan was administered at bedtime, which resulted in 75% of the patients reverted to dippers, a significant increase in the percentage of patients with controlled blood pressure after treatment, and a significant reduction in urinary albumin excretion.

This reduction was independent of the 24h blood-pressure decrease after treatment, but highly correlated with the decrease in the nocturnal mean of blood pressure, and mainly with the increase in day/night blood-pressure ratio associated to bedtime administration of valsartan.
