This is an excellent study of the differences in the composition of HCV quasispecies in plasma and circulating B cells of HCV patients. It analyzes the hypervariable region of the virus. The results provide a possible explanation for the chronicity of HCV infection and the development of autoimmune and lymphoproliferative processes in HCV patients.

The authors investigated the molecular mechanisms of interaction between HCV and HIV in mononuclear cells from co-infected individuals, offering possible molecular explanations for the frequent detection of autoimmune and lymphoproliferative processes in patients with HCV-HIV co-infection.

Clin Infect Dis 2004; 39:790-796. This large Taiwanese population-based study was performed in a community with a high (17%) prevalence of HCV infection. The frequency of thrombocytopenia in HCV patients was 10 times higher than in seronegative control individuals and increased in tandem with the severity of the HCV-related disease.

This study suggests the possible existence of false-negative results in the ELISA detection of anti-HCV antibodies in NHL patients, although confirmation in larger series of patients from other geographic areas is needed.
81. Castillo I, Pardo M, Bartolome J, et al. Occult hepatitis C virus infection in patients in whom the etiology of persistently abnormal results of liver-function tests is unknown. J Infect Dis 2004; 189:7-14. The authors studied 100 liver biopsy specimens from patients with altered liver function of unknown cause (in whom serum anti-HCV antibodies and RNA-HCV were negative) and detected HCV-RNA in 57 biopsy specimens. The virus was also detected in the circulating blood cells of 40 of these 57 patients. This study suggests the possible existence of an occult intracellular HCV infection in more than 50% of HCV- seronegative patients with a cryptogenetic liver disease.

This excellent editorial centers on analyzing the relation between chronic stimulation of B cells induced by chronic viral infections such as HCV and the development of autoimmune and lymphoproliferative disorders.

The authors describe a close association between the occurrence of B cell clonal expansion and extrahepatic manifestations in HCV patients.

The existence of circulating and liver-infiltrating monoclonal B cells in HCV patients is associated with a longer duration of HCV infection, cryoglobulinemic features, and the development of B cell lymphoma.
85. Machida K, Cheng KT, Sung VM, et al. Hepatitis C virus induces a mutator phenotype: enhanced mutations of immunoglobulin and protooncogenes. Proc Natl Acad Sci USA 2004; 101:4262-4267. The authors provide molecular evidence of the close association between HCV and cancer. They investigated the enhanced frequency of mutations in immunoglobulin genes and proto-oncogenes in HCV-infected cells. These two different oncogenic mechanisms may explain the Janus-like neoplastic expression observed in HCV patients with hepatocarcinoma and B cell lymphoma, the most frequently detected neoplasm in these patients.

88. Ambrosetti A, Zanotti R, Pattaro C, et al. Most cases of primary salivary mucosa-associated lymphoid tissue lymphoma are associated either with Sjogren syndrome or hepatitis C virus infection. Br J Haematol 2004; 126:43-49. This is a meticulous description of 33 patients with MALT lymphoma in the salivary glands, of which 73% were observed in patients with SS or HCV infection, underlining the sialotropism of the lymphomas occurring in patients with SS or HCV infection.

This is a good review (more etiopathogenic than clinical) of the use of anti-TNF agents in patients with chronic viral diseases, with two key messages: that there are very limited clinical data and that there is not yet adequate follow-up evaluating the possible long-term sequelae induced by the immunosuppression of anti-TNF blockade in these patients.
