Stroffolini T, Colloredo G, Gaeta GB, et al. Does an 'autoimmune' profile affect the clinical profile of chronic hepatitis C? An Italian multicentre survey. J Viral Hepat 2004; 11:257-262. This multicenter study involving 502 Italian HCV patients shows the lack of correlation between non-organ-specific antibodies and the main biochemical and histologic features, on one hand, and the response to antiviral therapy, on the other.

Frequent compart mentalization of hepatitis C virus variants in circulating B cells and monocytes
-
Ducoulombier D, Roque-Afonso AM, Di Liberto G, et al. Frequent compart mentalization of hepatitis C virus variants in circulating B cells and monocytes. Hepatology 2004; 39:817-825. This is an excellent study of the differences in the composition of HCV quasispecies in plasma and circulating B cells of HCV patients. It analyzes the hypervariable region of the virus. The results provide a possible explanation for the chronicity of HCV infection and the development of autoimmune and lymphoproliferative processes in HCV patients.

Occult hepatitis C virus infection in patients in whom the etiology of persistently abnormal results of liver-function tests is unknown
-
Castillo I, Pardo M, Bartolome J, et al. Occult hepatitis C virus infection in patients in whom the etiology of persistently abnormal results of liver-function tests is unknown. J Infect Dis 2004; 189:7-14. The authors studied 100 liver biopsy specimens from patients with altered liver function of unknown cause (in whom serum anti-HCV antibodies and RNA-HCV were negative) and detected HCV-RNA in 57 biopsy specimens. The virus was also detected in the circulating blood cells of 40 of these 57 patients. This study suggests the possible existence of an occult intracellular HCV infection in more than 50% of HCV- seronegative patients with a cryptogenetic liver disease.

Hepatitis C virus induces a mutator phenotype: Enhanced mutations of immunoglobulin and protooncogenes
-
Machida K, Cheng KT, Sung VM, et al. Hepatitis C virus induces a mutator phenotype: enhanced mutations of immunoglobulin and protooncogenes. Proc Natl Acad Sci USA 2004; 101:4262-4267. The authors provide molecular evidence of the close association between HCV and cancer. They investigated the enhanced frequency of mutations in immunoglobulin genes and proto-oncogenes in HCV-infected cells. These two different oncogenic mechanisms may explain the Janus-like neoplastic expression observed in HCV patients with hepatocarcinoma and B cell lymphoma, the most frequently detected neoplasm in these patients.

Safety of antitumour necrosis factor (anti-TNF) therapy in patients with chronic viral infections: Hepatitis C, hepatitis B, and HIV infection
-
Calabrese LH, Zein N, Vassilopoulos D. Safety of antitumour necrosis factor (anti-TNF) therapy in patients with chronic viral infections: hepatitis C, hepatitis B, and HIV infection. Ann Rheum Dis 2004;63(Suppl 2): ii18-ii24. This is a good review (more etiopathogenic than clinical) of the use of anti-TNF agents in patients with chronic viral diseases, with two key messages: that there are very limited clinical data and that there is not yet adequate follow-up evaluating the possible long-term sequelae induced by the immunosuppression of anti-TNF blockade in these patients.
