This is the largest prospective, randomized, controlled, multicentre trial on the use of AVP in out-of-hospital cardiac arrest. The effects of vasopressin were similar to those of epinephrine in the management of ventricular fibrillation and pulseless electrical activity, but vasopressin was superior to epinephrine in patients with asystole. Vasopressin followed by epinephrine may be more effective than epinephrine alone in the treatment of refractory cardiac arrest.

Arterial blood pressure 5 min after the intervention was significantly higher after vasopressin than with saline placebo or fluid resuscitation. Vasopressin improved abdominal organ blood flow but did not cause further blood loss. Seven out of seven vasopressin pigs survived until bleeding was controlled and 60 min thereafter, whereas seven out of seven saline placebo and seven out of seven fluid resuscitation pigs died.

This study showed the efficacy of terlipressin in the treatment of hypotension episodes in anaesthetized patients chronically treated with renin-angiotensin system inhibitors, angiotensin converting-enzyme inhibitors, and angiotensin II receptor antagonists. However, the negative effects on gastric mucosal perfusion and the risk of iatrogenic oxygen supply dependency of terlipressin need to be taken into account.
