This important report compared mortality risk as a function of pretransplant MELD score. Liver transplant survival benefit at 1 year is concentrated among patients at higher risk of pretransplant death. At MELD scores below 14, recipient mortality risk during the first posttransplant year was much higher than for those who remained on the transplant waiting list.

The investigators assessed the health status of patients with end-stage liver disease using validated disease-specific instruments and preference-based tools in outpatients with end-stage liver disease. In both cases HRQL was compromised in patients awaiting liver transplantation, but health status measures did not serve as proxies for utilities.

This retrospective review at a single center used the Karnofsky functional performance index and the Medical Outcomes Study Short Form 36 questionnaire to show that black patients had comparable survival and improvement in functional performance and HRQL with that of white patients. Although this study is important, its design had significant limitations.

This is the only prospective longitudinal study of HRQL in pediatric liver transplant recipients. The study was performed at a single center and showed that the greatest increase in HRQL occurred in the first 6 months after transplantation.
