This is a paper of outstanding relevance, which should be read by anyone who deals with pelvic organ prolapse. The authors collected all the available evidence concerning surgery of pelvic organ prolapse, providing meta-analysis of RCTs, whenever possible. The paper underscored the limits of the currently published studies, suggesting the objectives of further research. Methodologically perfect, however, it might be hard to go through for a reader not too expert in statistics.
14. Maher C, Qatawneh AM, Dwyer PL, et al. Abdominal sacral colpopexy or vaginal sacrospinous colpopexy for vaginal vault prolapse: A prospective randomized study. Am J Obstet Gynecol 2004; 190:20-26. This describes an RCT of excellent quality, well designed and powered according to a realistic estimation of the surgery success rate. The surgical techniques were adequately standardized, and the authors used validated questionnaires to assess health-related quality of life and lower urinary tract symptoms. Statistical analysis was appropriate. An example to follow for further studies. The main drawback of the study was the follow-up, which is only 24 months.

A randomized comparison of tension-free vaginal tape and endopelvic fascia plication in women with genital prolapse and occult stress urinary incontinence. Am J Obstet Gynecol 2004; 190:609-613. The paper deserves to be highlighted because it was one of the selected RCTs. The authors showed that TVT outperformed endopelvic iascia plication in treating occult stress urinary incontinence.
16. Nieminen K, Hiltunen KM, Laitinen J, et al. Transanal or vaginal approach to rectocele repair: a prospective, randomized pilot study. Dis Colon Rectum 2004; 47:1636-1642. The paper deserves to be highlighted because it was one of the selected RCTs. The authors compared transanal and vaginal techniques for rectocele repair, showing a higher prolapse recurrence rate with the transanal approach.
17. Roovers JPWR, van der Vaart CH, van der Bom JG, et al. A randomised controlled trial comparing abdominal and vaginal prolapse surgery: effects on urogenital function. Br J Obstet Gynaecol 2004; 111:50-56. This paper deserves to be highlighted because it was one of the selected randomized clinical trials. The authors suggested that vaginal hysterectomy with anterior or posterior colporrhaphy was preferable to abdominal sacrocolpopexy with preservation of the uterus in patients with uterine prolapse stages II-IV.
