One of our favourite papers of the year. It shows that well thought out protocols that are adhered to can make a major difference to outcome. It is a reminder that simple enforcement of existing policies will often dramatically produce improved results.
4. Karpati P, Rossignol M, Pirot M, et al. High incidence of myocardial ischaemia during postpartum hemorrhage. Anesthesiology 2004; 100:30-36. A very important paper that shows prolonged resuscitation can cause myocardial damage. Will this become a routine test for all major postpartum haemorrhage cases?
5. Bloom I, Verstang A, Giechinsky Y, et al. Arterial embolisation for persistent primary postpartum haemorrhage; before or after hysterectomy? Br J Obstet Gynaecol 2004; 111:880-884. An important retrospective study demonstrating the need to consider arterial embolization prior to hysterectomy for bleeding as it is associated with less complications.

Amniotic fluid embolism causing catastrophic pulmonary vasoconstriction: diagnosis by transesophageal echocardiogram and treatment by cardiopulmonary bypass. Obstet Gynecol 2003; 102:496-498. An important article demonstrating the usefulness of echocardiography to help define the diagnosis. It also shows that cardiopulmonary bypass can have a life-saving role in this condition.
