In August 1999 the Medicare Payment Advisory Commission (MedPAC) recommended a fundamental hospital payment change that would have combined Medicare's graduate medical education (GME) payments to teaching hospitals into a single teaching hospital payment adjustment that it argued would better account for the higher costs of inpatient care in those facilities. Although a major reform and not implemented, it would not have addressed directly the incentives issue discussed here.
