Solomon BL, Schaaf M, Smallridge RC: Psychologic symptoms before and after parathyroid surgery. Am J Med 1994, 96:101-106. Patients in the hyperparathyroid group demonstrated a greater number and intensity of distressful symptoms, which included interpersonal sensitivity, depression, anxiety, and hostility. The greatest improvement in symptoms occurred 1 month following surgery. Multidimensional psychological testing results were similar to those of control subjects after this time.

Surgery 1995, 117:392-396. The authors report a favorable outcome in more than 95% of 173 patients undergoing bilateral exploration. Preoperative localization studies were not performed. Biopsies were not performed on normal-appearing glands. Only one patient demonstrated postoperative hypoparathyroidism.

This excellent review of the literature shows asymptomatic or minimal hyperparathyroidism is not different from symptomatic hyperparathyroidism. It underlines the necessity of surgical intervention.

Preoperative ultrasound decreased the mean operating time from 113 to 97 minutes (P < 0.05) in this study. Costs were equivalent because of the cost of the ultrasound. All patients underwent bilateral neck exploration. If a unilateral approach had been used, cost savings may have been significant.

Am Surg 1995, 61:882-888. Sestamibi scintigraphy identified 10 of 12 patients with solitary adenomas confirmed by bilateral exploration. Hyperplasia was also successfully identified. Ultrasound in this study was a poor indicator.
