This paper provides extensive clinical and treatment information on a large cohort of patients with biopsy-proven temporal arteritis followed clinically over 27 years, including the absence of a decrease in cumulative steroid dose or visual outcome in patients receiving intravenous steroids compared with patients receiving oral steroids alone. The literature-based discussion on the reasons for different therapeutic approaches and clinical criteria for diagnosing giant cell arteritis between neuroophthalmologists and rheumatologists also brings into perspective the profound visual morbidity which can occur with giant cell arteritis.

Posterior circulation stroke via involvement of the vertebral arteries is an increasingly recognized manifestation of temporal arteritis. The necessity of early steroid treatment underscores the importance of early recognition. Posterior circulation stroke in an elderly patient, particularly in the absence of vascular risk factors, should immediately raise suspicion of temporal arteritis, and ESR and CRP should be immediately obtained, and temporal artery biopsy considered early.

55. Hall JK, Volpe NJ, Galetta SL, et al. The role of unilateral temporal artery biopsy. Ophthalmology 2003; 110:543-548. The necessity of bilateral versus unilateral temporal artery biopsy is a topic surrounded by controversy. This retrospective interventional case series of 181 patients assessed the role of unilateral temporal artery biopsy by evaluating for the presence of adverse outcomes with unilateral biopsy, which had a very low occurrence rate. The authors suggest specific clinical situations for which bilateral, sequential biopsies should be performed.
