The fellow eye in NAION: Report from the ischemic optic neuropathy decompression trial follow-up study
-
Newman NJ, Scherer R, Langenberg P, et al. The fellow eye in NAION: report from the ischemic optic neuropathy decompression trial follow-up study. Am J Ophthalmol 2002; 134:317-328. This paper presents very important 5-year data gleaned from the IONDT, which is a landmark study of NAION. It is the largest prospective trial of NAION, with invaluable information regarding the natural history of the disease, the risk of second eye involvement, and the factors which increase that risk.

-
Deramo VA, Sergott RC, Augsburger JJ, et al. Ischemic optic neuropathy as the first manifestation of elevated cholesterol levels in young patients. Ophthalmology 2003; 110:1041-1045. The importance of evaluating cholesterol levels in young patients with NAION in the absence of known vascular risk factors is underscored in this paper. Detection of elevated cholesterol may allow for effective intervention to decrease future vascular disease.

Management of giant cell arteritis: Our 27-year clinical study: New light on old controversies
-
Hayreh SS, Zimmerman B. Management of giant cell arteritis: our 27-year clinical study: new light on old controversies. Ophthalmologica 2003; 217:239-259. This paper provides extensive clinical and treatment information on a large cohort of patients with biopsy-proven temporal arteritis followed clinically over 27 years, including the absence of a decrease in cumulative steroid dose or visual outcome in patients receiving intravenous steroids compared with patients receiving oral steroids alone. The literature-based discussion on the reasons for different therapeutic approaches and clinical criteria for diagnosing giant cell arteritis between neuroophthalmologists and rheumatologists also brings into perspective the profound visual morbidity which can occur with giant cell arteritis.

Bilateral vertebral artery occlusion resulting from giant cell arteritis: Report of 3 cases and review of the literature
-
Rüegg S, Engelter S, Jeanneret C, et al. Bilateral vertebral artery occlusion resulting from giant cell arteritis: report of 3 cases and review of the literature. Medicine 2003; 82:1-12. Posterior circulation stroke via involvement of the vertebral arteries is an increasingly recognized manifestation of temporal arteritis. The necessity of early steroid treatment underscores the importance of early recognition. Posterior circulation stroke in an elderly patient, particularly in the absence of vascular risk factors, should immediately raise suspicion of temporal arteritis, and ESR and CRP should be immediately obtained, and temporal artery biopsy considered early.

The role of unilateral temporal artery biopsy
-
Hall JK, Volpe NJ, Galetta SL, et al. The role of unilateral temporal artery biopsy. Ophthalmology 2003; 110:543-548. The necessity of bilateral versus unilateral temporal artery biopsy is a topic surrounded by controversy. This retrospective interventional case series of 181 patients assessed the role of unilateral temporal artery biopsy by evaluating for the presence of adverse outcomes with unilateral biopsy, which had a very low occurrence rate. The authors suggest specific clinical situations for which bilateral, sequential biopsies should be performed.
