This paper examines outcome in three domains in children with congenital hypothyroidism identified by screening. Highlighted are a weakness in visuospatial processing that is associated with a prenatal thyroid hormone insufficiency, selective memory deficits associated with a postnatal thyroid hormone insufficiency, a sensorimotor weakness also associated with a postnatal insufficiency, and attention deficits reflecting abnormal thyroid hormone levels at time of testing.

Results from infants with congenital hypothyroidism tested on four occasions between 3 and 18 months were submitted to regression analysis examining aspects of fetal hypothyroidism, initial severity, treatment age, dosage, and efficacy of therapy. Results found that fetal and neonatal hypothyroidism have different effects on brain maturation sites during the first 2 years and some deficits are likely unavoidable.
