Serum antibody markers of tick exposure were low at baseline and did not significantly differ between intervention and control participants during the follow-up.

The Back Home Trial. General practitioner-supported leaflets may change back pain behavior
-
Roberts L, Little P, Chapman J, et al.: The Back Home Trial. General practitioner-supported leaflets may change back pain behavior. Spine 2002, 27:1821-1828. Investigators used extensive formative research to develop an educational leaflet on management of low back pain and randomized community-based general practitioners to either encourage their patients with low back to read the leaflet or provide usual care. This relatively simple intervention resulted in significant changes in patients' behaviors that could prevent further episodes of low back pain.

Rapoff MA, Belmont J, Lindsley C, et al.: Prevention of nonadherence to non-steroidal anti-inflammatory medications for newly diagnosed patients with juvenile rheumatoid arthritis. Health Psychol 2002, 21:620-623. In this RCT, patients with newly diagnosed juvenile rheumatoid arthritis who were randomized to an educational plus behavioral intervention had significantly better adherence than patients who received standard education on medication adherence. Adherence was measured using specialized electronic medication bottle caps that recorded the dates and times the patient opened the medication bottle.

-
Dally DL, Dahar W, Scott A, et al.: The impact of a health education program targeting patients with high visit rates in a managed care organization. Am J Health Promot 2002, 17:101-111. In this RCT, intervention patients with arthritis, hypertension, or diabetes received mailings of tailored, disease-specific, self-management education and of personalized feedback. The most significant improvements occurred among patients with arthritis. This relatively low-cost intervention could potentially generate a large cost savings for the health system.

Weight from birth to 53 years. A longitudinal study of the influence on clinical hand arthritis
-
Sayer AA, Poole J, Cox V, et al.: Weight from birth to 53 years. A longitudinal study of the influence on clinical hand arthritis. Arthritis Rheum 2003, 48:1030-1033. This long-term, longitudinal study presents the first published evidence that low birth weight may contribute to the development of hand OA among men. These observations indicate that factors related to the prenatal environment may have an influence on adult joint development and function.

-
Tench CM, McCarthy J, McCurdle I, et al.: Fatigue in systemic lupus erythematosus: a randomized controlled trial of exercise. Rheumatology 2003, 42:1050-1054. In this RCT, participants in a 12-week graded exercise program experience significantly improved fatigue; for those who continued to exercise after the intervention period, fatigue scores continued to improve. These results suggest that appropriately graded exercise is efficacious in the managing disease-related fatigue among patients with SLE.

-
Rejeski WJ, Focht BC, Messier SP, et al.: Obese, older adults with knee osteoarthritis: weight loss, exercise, and quality of life. Health Psychol 2002, 21:419-426. The Arthritis, Diet and Activity Promotion Trial examines the independent and combined effects of exercise and diet on the quality of life of obese, older adults with documented knee OA. Over the 18-month follow-up, subjects in the diet, exercise, and diet plus exercise groups lost significantly more (2.6%-5.7% vs 1.3% for controls) body weight and reported significant improvements in quality-of-life measures.

-
Thomas KS, Muir KR, Doherty M, et al.: Home based exercise programme for knee pain and knee osteoarthritis: a randomised controlled trial. BMJ 2003, 325:752-756. This study used a factorial design to randomize 786 subjects with self-reported knee pain to four groups: exercise plus telephone contact, exercise, telephone contact, and placebo. Compared with the nonexercise groups (telephone contact, placebo), the exercise groups (with or without telephone contact) showed decreased knee pain and improved physical function. Adherence to exercise had a dose-response relationship with pain improvement; telephone contact alone did not appear to decrease pain.

Baseline balance and functional decline in older adults with knee pain: The observational arthritis study in seniors
-
Marsh AP, Rejeski WJ, Lang W, et al.: Baseline balance and functional decline in older adults with knee pain: the observational arthritis study in seniors. J Am Geriatr Soc 2003, 51:331-339. This longitudinal study investigates the association between individuals' baseline balance with self-reported disability and functional measurements, independent of muscular strength. Better balance at baseline was associated with less decline in function at 30 months.

Quadriceps strength and osteoarthritis progression in malaligned and lax knees
-
Sharma L, Dunlop DD, Cahue S, et al.: Quadriceps strength and osteoarthritis progression in malaligned and lax knees. Ann Intern Med 2003, 138:613-618. This prospective, longitudinal cohort study indicates that persons with baseline knee malalignment or with ligament laxity and high quadriceps strength may have an increased risk of progression of tibiofemoral OA. These findings provide a cautionary note regarding recommendations for quadriceps strengthening exercises among this subgroup of patients with OA.

Population-based randomized controlled trial of a stage-targeted physical activity intervention
-
Marshall AL, Bauman AE, Owen N, et al.: Population-based randomized controlled trial of a stage-targeted physical activity intervention. Ann Behav Med 2003, 25:194-202. Results from this large, community-based RCT suggest that a relatively low-intensity, stage-targeted, mailed intervention program may result in short-term (3-6 months) improvements in physical activity levels. These results may have practical applications as primary and tertiary prevention strategies for rheumatic disease.

Computer use and carpal tunnel syndrome. A 1-year follow-up study
-
Andersen JH, Thomsen JF, Overgaard E, et al.: Computer use and carpal tunnel syndrome. A 1-year follow-up study. JAMA 2003, 289:2963-2969. This longitudinal cohort study included 9480 workers at 3500 workplaces with a wide distribution of mouse devices and keyboard use. The prevalence of possible CTS was 4.8%, and the incidence of new symptoms was 5.5%. If the investigators had used diagnostic tests to define the presence of CTS, these estimates would likely have been lower.

Secondary prevention of work-related disability in nonspecific low back pain: Does problem solving therapy help? A randomized clinical trial
-
van den Hout JHC, Vlaeyen JWS, Heuts PHTG, et al.: Secondary prevention of work-related disability in nonspecific low back pain: does problem solving therapy help? A randomized clinical trial. Clin J Pain 2003, 19:87-96. Study participants with low back pain were recruited from a rehabilitation center and were randomized to graded activity/group education plus problem-solving therapy (GAPS) or GAGE. The GAPS program resulted in more employees with a 100% return-to-work status and fewer employees who received disability pensions 12 months post-intervention.

Psychologically based occupational rehabilitation: The pain-disability prevention program
-
Sullivan MJL, Stanish WD: Psychologically based occupational rehabilitation: the pain-disability prevention program. Clin J Pain 2003, 19:97-104. This paper describes the development, implementation, and preliminary findings of a cognitive-behavioral intervention for pain-related occupational injuries. Participants included 104 claimants to the Worker's Compensation Board of Nova Scotia. A network of community psychologists assisted in delivering the program, which resulted in 60% treatment success.

Active neck muscle training in the treatment of chronic neck pain in women. A randomized controlled trial
-
Ylinen J, Takala EP, Nykanen M, et al.: Active neck muscle training in the treatment of chronic neck pain in women. A randomized controlled trial. JAMA 2003, 289:2509-2516. Female office workers with chronic neck pain were randomized to strength training, endurance training, or usual care. Compared with the control group, the strength training group showed significant increases in range of motion in all directions, whereas the endurance training group showed significant changes in neck rotation. Although all groups decreased their use of medications, a greater reduction in medication use occurred among the strength training and endurance training groups.
