ARDS Clinical Trials Network, National Heart, Lung and Blood Institute, National Institutes of Health: Effects of recruitment maneuvers in patients with acute lung injury and acute respiratory distress syndrome ventilated with high positive end expiratory pressure
-
Brower RG, Morris A, MacIntyre N, et al., ARDS Clinical Trials Network, National Heart, Lung and Blood Institute, National Institutes of Health: Effects of recruitment maneuvers in patients with acute lung injury and acute respiratory distress syndrome ventilated with high positive end expiratory pressure. Crit Care Med 2003; 31:2592-2597. In this article, the authors observed that short-term effects of recruitment maneuvers are variable in ALI/ARDS patients receiving mechanical ventilation with low tidal volumes and high PEEP. Beneficial effects on gas exchange in responders appear to be brief, demonstrating that higher PEEP levels could be needed to sustain the effects of recruitment maneuvers.

Alveolar recruitment in combination with sufficient positive end expiratory pressure increases oxygenation and lung aeration in patients with severe chest trauma. Crit Care Med 2004; 32:968-975. Regional lung volume during high-frequency oscillatory ventilation by electrical impedance tomography
-
Van Genderingen HR, Van Vught AJ, Jansen JR: Regional lung volume during high-frequency oscillatory ventilation by electrical impedance tomography. Crit Care Med 2004; 32:787-794. In this article, the authors showed a more homogenous regional lung volume distribution with high-frequency ventilation. However, thoracic fluid accumulation may complicate its interpretation.

Standardized lung recruitment dur9 ing high frequency and conventional ventilation: Similar pathophysiologic and inflammatory responses in an animal model of respiratory distress syndrome
-
Krishnan RK, Meyers PA, Worwa C, et al. Standardized lung recruitment dur9 ing high frequency and conventional ventilation: similar pathophysiologic and inflammatory responses in an animal model of respiratory distress syndrome. Intensive Care Med 2004; 30:1195-1203. In this article the authors concluded that using a standardized lung recruitment strategy in spontaneously breathing animals, volume targeted conventional ventilation and HFO produced equivalent outcomes.

Morphometric effects of the recruitment maneuver on saline-lavaged canine lungs: A computed tomographic analysis
-
Lim CM, Soon Lee S, Seoung Lee J, et al. Morphometric effects of the recruitment maneuver on saline-lavaged canine lungs: a computed tomographic analysis. Anesthesiology 2003; 99:71-80. In this article, the authors studied the morphometric evaluation of recruitment maneuvers plus PEEP compared with PEEP titration without recruitment maneuvers. They concluded that recruitment maneuvers resulted in a greater lung volume, better aeration of the most dependent lung, and less regional heterogeneity of aeration. However, the recruitment maneuvers tended to induce a greater increase in hyperaerated lung volume.

Ventilatory management of acute respiratory distress syndrome: A consensus of two
-
Marini JJ, Gattinoni L: Ventilatory management of acute respiratory distress syndrome: a consensus of two. Crit Care Med 2004, 32:250-255. The authors reviewed the relevant scientific literature and personal observations from clinical practice about ventilatory management of ARDS. They concluded that the prevention of ventilator-induced lung injury and ventilatory strategies, including recruitment maneuvers and titration of PEEP, are a complex undertaking that requires application of principles founded on a broad experimental and clinical database and the results of well-executed clinical trials.

Effects of positive end expiratory pressure on lung recruited volume and oxygenation in patients with acute respiratory distress syndrome

Qiu HB, Xu HY, Yang Y, et al. Effects of positive end expiratory pressure on lung recruited volume and oxygenation in patients with acute respiratory distress syndrome. Zhongguo Wei Zhong Bing Ji Yi Xue 2004; 16:399-402. In this article, the authors studied 11 ARDS patients and concluded that recruited volume increases with PEEP, and the PEEP-induced increase in arterial oxygenation is significantly correlated to recruited volume.

Clinical impact of recruitment maneuvers in patients with acute respiratory distress syndrome
-
Oczenski W, Schwarz S, Fitzgerald RD: Clinical impact of recruitment maneuvers in patients with acute respiratory distress syndrome. Anaesthesiol Intensivmed Notfallmed Schmerzther 2004; 39:463-470. In this article, the authors concluded that successful recruitment depends on several factors like the applied recruitment pressure, the level of PEEP set before and after the recruitment maneuver, the stage and the underlying disease of ARDS, chest wall mechanics, transpulmonary pressure, and the positioning of the patient.
