Thompson WW, Shay DK, Weintraub E, et al. Mortality associated with influenza and respiratory syncytial virus in the United States. JAMA 2003; 289:179-186. Large population-based study describing that mortality is associated with the circulation of both influenza and respiratory syncytial virus, and disproportionately affects the elderly. Asthma and influenza vaccination

Eisner MD. Asthma and influenza vaccination. Chest 2003; 124:775-777. Excellent editorial summarizing the (lack of) evidence, the problems and possible reasons for low coverage of vaccination for asthma patients Interesting plea from the author to embrace the available indirect evidence for the probable benefits of influenza vaccination in asthma patients. He recalls the strong evidence for its safety and finds further trials unnecessary.

Bueving HJ, Bernsen RM, de Jongste JC, et al. Influenza vaccination in children with asthma: randomized double-blind placebo-controlled trial. Am J Respir Crit Care Med 2004; 169:488-493. Interesting randomized double-blind, placebo-controlled trial in children aged 6-18 years with asthma on the clinical effectiveness of influenza vaccination. The first study taking culture confirmed asthma exacerbations, based on symptom scores, as an endpoint. Study was set up to detect a 50% reduction in the number of asthma exacerbations. Confidence interval excludes a 35% reduction. Low influenza activity during the first season.

DOI: 10.1002/14651858. CD000364.pub2. This high-quality, extensive, systematic review provides an overview of the best evidence available. No restrictions for language were used. Only randomized trials were included. No increase in asthma exacerbations immediately after vaccination was found. No firm evidence from controlled trials was found to support the adoption of universal vaccination in patients with asthma as a clinical policy.

DOI: 10.1002/14651858. CD001269.pub2. Extensive systematic review of high quality. No restrictions for language were used. Only randomized trials of influenza vaccination in adults were included. It was concluded that influenza vaccines are effective in reducing serologically confirmed cases of influenza. However, they are not as effective in reducing cases of clinical influenza.

Excess hospital admissions for pneumonia, chronic obstructive pulmonary disease, and heart failure during influenza seasons in Hong Kong
-
Yap FH, Ho PL, Lam KF, et al. Excess hospital admissions for pneumonia, chronic obstructive pulmonary disease, and heart failure during influenza seasons in Hong Kong. J Med Virol 2004; 73:617-623. Intriguing because no association was found for influenza activity as an independent risk factor for admission rates in asthma.

Effectiveness of influenza vaccine for the prevention of asthma exacerbations
-
Christy C, Aligne CA, Auinger P, et al. Effectiveness of influenza vaccine for the prevention of asthma exacerbations. Arch Dis Child 2004; 89:734-735. Interesting because of the totally unexpected findings in a retrospective cohort study of 800 children with asthma. After adjusting for other variables, the vaccine group had a significantly increased risk of asthma-related emergency department and clinic visits (odds ratio 1.9 and 3.4, respectively).

Influenza vaccination in patients with asthma: Effect on the frequency of upper respiratory tract infections and exacerbations
-
Abadoglu O, Mungan D, Pasaoglu G, et al. Influenza vaccination in patients with asthma: effect on the frequency of upper respiratory tract infections and exacerbations. J Asthma 2004; 41:279-283. Nice and simple randomized, double-blind, placebo-controlled trial in adults. No effect on asthma exacerbations was found, even in the subgroup with severe asthma. No cultures were taken during exacerbations.

Conventional influenza vaccination is not associated with complications in working-age patients with asthma or chronic obstructive pulmonary disease
-
Hak E, Hoes AW, Grobbee DE, et al. Conventional influenza vaccination is not associated with complications in working-age patients with asthma or chronic obstructive pulmonary disease. Am J Epidemiol 2003; 157: 692-700. Large-scale study. First, the results were analysed in a nested case control design. No associations were found between influenza vaccination and reduction of complications. Second, application of a new method (propensity score) was performed to further remove confounding by indication. No association was found.
