The recent publication of this multicentre trial has supported the efficacy of carotid endarterectomy in preventing stroke in patients with asymptomatic moderate ar severe internal carotid artery stenosis. The stroke rate with surgery was 3.1%, although the benefit in preventing stroke at five year follow up was confined to those under the age of 75 with a good expectation of medium term survival.

This early trial of carotid angioplasty showed similar outcomes in the surgical and angioplasty groups, although it has been criticised for much higher stroke rates than in the larger carotid endarterectomy trials. A second CAVATAS trial, incorporating advances in stent and distal embolic protection technology, is ongoing.
