This is a retrospective, registry-based review article that evaluates the significance of symptomatic PE in patients with head injury trauma. The results suggest that head injury is not a risk factor for PE and conflict with results in other contemporary literature.

This retrospective review of one spinal cord injury center's 3-year experience with respect to VTE. Although it defines the risk factors for VTE in the population with spinal cord injury, there are issues with patient selection techniques, and the data must be carefully scrutinized.

This well-done evaluation of more than 1000 trauma patients at high risk of VTE demonstrated no difference between lower extremity or pelvic fracture and immobility alone as risk factors for DVT and PE.

The authors attempt to define the incidence of DVT and PE in the critically ill surgical patient using an intensive surveillance protocol with duplex ultrasonography.

J Trauma 2003, 55:1162-1167. This prospective observational study used duplex ultrasonography to establish the prevalence of lower extremity DVT in burned patients. The independent risk factors for the development of VTE were also examined.

This retrospective review of 92 patients with traumatic spinal cord injury who had admission screening Doppler ultrasound examinations aimed to establish the prevalence of DVT in this population.
20. Kadyan V, Clinchot DM, Colachis SC: Cost-effectiveness of duplex ultrasound surveillance in spinal cord injury. Am J Phys Med Rehabil 2004, 83:191-197. This study of patients admitted for traumatic spinal cord injury examines the cost-effectiveness of surveillance Doppler ultrasound screening for DVT in the population with spinal cord injury. This elegant analysis compares the cost-effectiveness analysis for screening duplex with that of breast, cervical, and colon cancer and other widely accepted forms of screening and reports a favorable comparison.
21. Schultz DJ, Brasel KJ, Washington L, et al.: Incidence of asymptomatic pulmonary embolism in moderately to severely injured trauma patients. J Trauma 2004, 56:727-733. This prospective cohort study examines the rate of asymptomatic PE in patients with an injury severity score >8, using contrast-enhanced helical CT scan. The rates of silent PE and DVT are examined. Risk factors for PE post-injury were identified and concerns were raised about the adequacy of prophylaxis regimens, including LMWH and low-dose heparin.
22. Velmahos GC, Toutouzas KG, Vassiliu P, et al.: Can we rely on computed tomographic scanning to diagnose pulmonary embolism in critically ill surgical patients? J Trauma 2004, 56:518-526. This prospective study compares the gold standard for detection of PE, pulmonary angiography, with an emerging technology, CTA, in critically ill surgical patients with clinical suspicion of PE. This study brings into question the reliability of CTA for this purpose.

24. Ginzburg E, Cohn SM, Lopez J, et al.: Randomized clinical trial of intermittent pneumatic compression and low molecular weight heparin in trauma. Br J Surg 2003, 90:1338-1344. The authors compare LMWH and SCDs in a randomized clinical trial but are unable to establish the superiority of either modality as a method of prophylaxis for VTE in trauma patients. They confirm the safety of LMWH in this population.

Spinal Cord Injury Thromboprophylaxis Investigators: Prevention of venous thromboembolism in the acute treatment phase after spinal cord injury: a randomized, multicenter trial comparing low-dose heparin plus intermittent pneumatic compression with enoxaparin. J Trauma 2003, 54:1116-1126. This well-done, prospective, randomized, controlled trial compares low-dose heparin and SCDs with LMWH. Rates of DVT by venography and ultrasonography as well as rates of PE were examined.
26. Murakami M, McDill TL, Cindrick-Pounds L, et al.: Deep venous thrombosis prophylaxis in trauma: improved compliance with a novel miniaturized pneumatic compression device. J Vasc Surg 2003, 38:923-927. The authors demonstrate that patient compliance is improved with a miniaturized pneumatic compression device compared with traditional SCDs.
27. Alejandro KV, Acosta JA, Rodriguez PA: Bleeding manifestations after early use of low-molecular-weight heparins in blunt splenic injuries. Am Surg 2003, 69:1006-1009. A well-done retrospective, chart review study examines the rate of bleeding in patients with splenic injuries, receiving LMWH for DVT prophylaxis and confirms the safety of LMWH in select patients with solid organ injury.
28. Chiou-Tan FY, Garza H, Chan KT, et al.: Comparison of dalteparin and enoxaparin for deep venous thrombosis prophylaxis in patients with spinal cord injury. Am J Phys Med Rehabil 2003, 82:678-685. This thorough prospective, randomized trial compares the efficacy of dalteparin and enoxaparin for prophylaxis of DVT in patients with spinal cord injury. The efficacy of both agents is confirmed. Of note is the survey portion of the article, which demonstrates the benefit of dalteparin's once-daily dosing regimen over enoxaparin's twice-daily dosing regimen.
