Excellent review of juvenile DM with a focus on epidemiology and proposed mechanisms for pathogenesis, including infectious agents, genetic polymorphisms, and immune-mediated mechanisms.
25. Werth VP, Callen JP, Ang G, et al.: Associations of tumor necrosis factor alpha and HLA polymorphisms with adult dermatomyositis: implications for a unique pathogenesis. J Invest Dermatol 2002, 119:617-620. Evaluation of 50 adult patients with DM and 239 controls demonstrated increased incidence (27 vs 14%, respectively) of the TNF-α 308A in DM, but no association with HLA-DR3 as previously demonstrated in patients with subacute cutaneous lupus erythematosus. Possession of the TNF-α 308A polymorphism leads to increased TNF-α production and increased keratinocyte ultraviolet-induced apoptosis, which may be pathogenic for the photosensitivity in DM. These findings may have future implications in treatment modalities using the TNF-α inhibitors.

Mannose binding lectin, which is involved in clearance of apoptotic keratinocytes, may also be involved in photosensitivity; increased polymorphism of mannose binding lectin was demonstrated in patients with DM with polymorphisms of the TNF-α 308A allele.

Lancet 2002, 360:1197-1202. Evaluation of a juvenile population in the West Midland region of the UK to compare the incidence of vasculitides and connective tissue disease among ethnic groups showed 14 patients with juvenile DM. The study calculated a relative risk of 10 for juvenile DM in blacks as compared to whites, but this study is limited by a small number in the juvenile DM group.
