The Cape regulations of 1810 set midwives' fees at 15 Rixdollars (or Rds) (£2 12s 6d) for attending a (white) mistress and 5 Rds (173 6d) for attending a slave. This was much higher than the then current rate of fees for a visit by a physician, which was 1 Rd (33 6d) and exceeded the lowest rate for man-mid wives in 1830 (£2).

Although not compulsory, nearly 40 percent of Edinburgh M.D. graduates between 1763 and 1826 had taken midwifery courses (L. Rosner, Medical Education in the Age of Improvement : Edinburgh Students and Apprentices, 1760-1826 [Edinburgh, 1991], 70-1). Local practitioners fought successfully for a chair in midwifery at the university in 1840 and founded the Edinburgh Obstetrical Society to raise their profile (Jenkinson, Scottish Medical Societies, 40-1).

"Called to her at three o'clock a.m.": Obstetrical practice in physician case notes
-
These midwifery clients represented about 5 per cent of his total clientele during the year. Rates of man-midwifery attendance by comparison, were 22 cases for a case load of 650 per annum (3-4 per cent of cases) for William Parker in America (A. M. Kass, '"Called to her at three o'clock a.m.": obstetrical practice in physician case notes', Journal of the History of Medicine and Allied Sciences, L (1995), 202). It should be remembered however that the 5 per cent figure for Chiappini represents a proportion of clients rather than visits.

Although Chiappini had many black clients, only four other midwifery cases throughout the account book involved black women, all probably Muslim, and in none of these was Chiappini present at the birth itself.
