2. The interviews were conducted between October 1995 and February 1996. Each interview was conducted by a team of two or three researchers, who followed a core protocol and added questions based on respondents' knowledge and responses to questions. The lead interviewer (Paul Ginsburg) was the same for all fifteen interviews, and a second researcher (Janet Corrigan) participated in thirteen interviews. Other researchers involved in the interviews were Peter Cunningham, Joy Grossman, Peter Kemper, and Linda Kohn of the Center for Studying Health System Change; Robert St. Peter of the center and Mathematica Policy Research; and Stephen Long and Susan Marquis of RAND.
