The algorithm defines patients as having diabetes if they (1) currently are taking or have taken in the past three years insulin or oral agents, (2) had a total glycosolated hemoglobin > 7.5 in the twelve months prior to the "pull date" (the date at which the registry is refreshed), (3) had a fasting plasma glucose > 140 in the twelve months prior to the pull date, (4) had a random plasma glucose > 200 in the twelve months prior to the pull date, or (5) had a hospital discharge diagnosis of diabetes at any time during their medical history with GHC.
