    • The survey defined each type of plan for the respondent as follows: Conventional health insurance (also called indemnity insurance or a fee-for-service plan): Under conventional insurance, employees can go to any physician or hospital they choose. The employee typically faces a deductible and coinsurance above the deductible. Health maintenance organization, or HMO: With an HMO, employees must receive their care from an HMO physician; otherwise the expense is not covered. When they use HMO physicians, however, expenses typically are covered in full with a flat-dollar copayment for the office visit. Preferred provider organization, or PPO: PPO plans typically are those in which employees have lower deductibles and coinsurance if they use selected doctors or hospitals within a network. Point-of-service plan (also called an HMO/PPO hybrid, open-ended HMO, or POS plan): This type of plan resembles an HMO for in-network services (that is, with copayments), while it reimburses employees for services they receive outside the network on a fee-for-service basis, often after a deductible and coinsurance.

It was reported in the article that the percentages of subscribers with a limit of $250,000 to $1 million were 283 in conventional plans and 18.9 in PPOs, and that the percentages with a limit of more than $1 million were 37.9 and 50.3, respectively. The correct labels for these statistics are actually "$250,000 to $999,999" and "$1 million or more."
